Office Policies
General
- In accordance with HIPAA privacy regulations, we do not fax medical records to unsecure, unattended lines, please arrange to pick up records.
- In accordance with HIPAA, no cell phone or recording device use are permitted in the office. Please turn your cell phone off while you are at the office.
- All medical forms take 5-7 business days to process. Please make sure that all forms being turned in are fully completed.
- You may receive secure text message reminders for your child(ren)'s appointments.
Cancellations
- Our office requires 24 hours' advanced notice for any cancellations. Excessive late cancellations may result in discharge from the practice.
- NO SHOWS may result in discharge from the practice.
- We do not discharge one child. We discharge the entire household/family.

- Lyndon Pediatrics accepts cash, checks, and the following major credit cards: Visa, Mastercard, and American Express.
- Deductible and co-payments are due at the time of service. The amount of your co-payment is determined by your insurance policy. A minimum deductible payment of $25 will be collected at the time of service with the rest billed to your insurance.
- If you do not have insurance, a minimal office visit fee of $100 will be collected before services are rendered. The remaining payment is due at the end of your appointment.
- The parent/guardian/adult accompanying the patient is responsible for any payment due at the time of service. PLEASE ARRANGE THAT THIS ADULT BRINGS PAYMENT DUE TO THE VISIT.
Appointments
- It is the policy of Lyndon Pediatrics that every child, depending on age, has at least one well child/preventative physcial each year.
- Please have all necessary forms filled out before your examination. Please arrive 5 minutes early to your appointment to fill out the necessary forms, or download the forms you need here.
- If you are coming in for a visit with one child, please call in advance if you want another child seen. We may not be able to accommodate your additional child(ren).
- It is our policy to not accept new patients who are nearing the age that they would require seeing a Family Practice or Internist as that is in the best interest of the patient.
Prescriptions
- New York State law requires prescriptions to be electronically sent to the pharmacy, INCLUDING CONTROLLED SUBSTANCES such as ADD/ADHD medication. Please allow up to 5 to 7 business days to prescribe controlled substances as they require special coding.
- Call at least one week before you are due to run out of your medication.
- Be prepared to give the office staff a complete request for the medication. This includes the medication name, prescription number, dosage, frequency, pharmacy name, pharmacy phone number, etc.
Immunizations
- Lyndon Pediatric Associates follows the immunization schedule recommended by the American Academy of Pediatrics.
- We DO NOT skip immunizations/vaccines. We do not delay vaccines. We do not follow an alternative vaccine schedule. We do not accept patients who refuse vaccines.
- A parent or legal guardian must be present for children to receive immunizations. If no parent or guardian is present, a child must have written permission from their parent or guardian. If no consent is obtained, an additional visit to complete vaccines will be required.
- An updated immunization form will be presented to patients at every wellness exam as feasibly possible.
