At your child's well visit, we understand that you may have concerns at that time. We have booked your time sufficient for a routine well visit. We respect everyone's time and ask that you make a separate appointment to address those concerns. Thank you.

Please arrive 10 minutes before a well-child visit and 5 minutes before a sick visit.

If a patient arrives 8 minutes LATE for a scheduled sick visit appointment, the appointment may need to be rescheduled. 

If a patient arrives 10 minutes LATE for a scheduled well-child visit, they will be required to reschedule their appointment for another time/day.

A no-show fee of $35 will be billed to you if you do not provide at least 24 hours notice before the cancellation of your scheduled appointment.

Medication shortages:

We apologize for the inconvenience. Due to nationwide shortages of some medications, they may not be available at your regular pharmacy. You may need to research the availability and we will fill it when you provide us with the complete information of the pharmacy that has the medication available. We appreciate your understanding during this time. Please see our office policy tab at the top of this page for refill policies. Refill requests are NOT available on the patient portal at this time. When calling in you MUST have the name and dosage of the medication from the bottle. Thank you.

** NOTE**: We do not see children, in the office, with the stomach bug. It is highly contagious. If you suspect that your child is dehydrated, please go to the emergency room. 

 

Parents or Legal Guardians:

If you are not accompanying your child/children to their appointment we must have permission, in writing, for the adult (18 years of age and older) that will be with them to give consent to evaluate and treat as well as administer any vaccine. The permission slip is available on our website! The adult with the patient is responsible for any payment that is due at the time of service. PLEASE ARRANGE THAT THE ADULT BRINGS PAYMENT (i.e. co-pay, deductible, coinsurance, past due balance).

DON'T LOSE COVERAGE

Excellus, Fidelis Child Health Plus, and Managed Medicaid patients! Please contact your plan to be sure that you are eligible for coverage. Their rules have changed due to the end of the COVID-19 Pandemic. If you have a change of address you must contact your plan at the number on the back of the ID card to update your information!
