Insurance Coverage Scams

Don't be fooled by "too good to be true coverage"!

If your family needs health insurance not available by your employer please go to this website to sign up: https://nystateofhealth.ny.gov/

Please confirm with our office that we participate with the coverage you are considering before you make your choice!

Well Child Visits

We understand that you may have concerns at your child's well visit. We have booked your time sufficiently for a "routine well visit". We respect everyone's time and ask that you make a separate appointment to address those concerns. Sometimes, the issue may be addressed at that time, requiring a separate billing code for your concern addressed. Please be advised that you will be responsible for any co-pay, co-insurance, or deductible due per your insurance benefits. Thank you. 

Please arrive 10 minutes before a well-child visit and 5 minutes before a sick visit.

If a patient arrives LATE for a scheduled sick visit appointment, the appointment may need to be rescheduled. You may also be required to wait until the provider can "work" you back into the schedule.

If a patient arrives LATE for a scheduled well-child visit, they will be required to reschedule their appointment for another time/day.

 

** NOTE**: We do not see children, in the office, with the stomach bug. It is highly contagious. If you suspect that your child is dehydrated, please go to the emergency room.

 

Medication Shortages

Due to nationwide shortages of some medications, the pharmacy may not have your medication in stock. If your medication is unavailable, you must research the availability and we will fill it when you provide our office with the complete information of the pharmacy that has the medication available.

Please refer to the office policies tab regarding medication refills. Refill requests are NOT available on the patient portal. When calling for a refill, you must have the medication name, dosage of the medication, frequency of dosing, pharmacy name, and pharmacy phone number for your request to be completed. Thank you!

 

Parents/Legal Guardians

If you are not accompanying your child(ren) to their appointment we must have permission, in writing, for an adult (18 years of age and older) that will be with them to give consent to evaluate and treat as well as administer any vaccine. The permission slip is available on our website! The adult with the patient is responsible for any payment that is due at the time of service. PLEASE ARRANGE THAT THE ADULT BRINGS PAYMENT (i.e. co-pay, deductible, coinsurance, past due balance).

Please have the adult bring a completed Authorization to Treat Minor form!

 

ATTENTION: Don't lose coverage!

Excellus, Fidelis Child Health Plus, and Managed Medicaid patients! Please contact your plan to be sure that you are eligible for coverage. Their rules have changed due to the end of the COVID-19 Pandemic. If you have a change of address you must contact your plan at the number on the back of the ID card to update your information!
